List the indexing terms IEEE Xplore holds for one paper, one result per vocabulary: the IEEE thesaurus terms, the index terms IEEE derives, the terms the authors supplied and, for medical content, MeSH terms
Each result is one vocabulary, named in `vocabulary`, with its terms in `terms` — the vocabularies overlap and are not a ranking, so treat them as separate lists rather than merging them. Courses and some older records are indexed by no vocabulary at all, so an empty result is a real answer.
